If I were planning a first trip

Rome → Florence → Bologna → Venice is an excellent 8-10 day route. They're also well connected by Italy's high-speed rail network; for example, Bologna is roughly an hour from Milan and about 1½ hours from Venice, while Florence-Rome can take about 1½ hours by high-speed train.

If you have two weeks, I'd add Naples and either Milan or a few days in Tuscany.

A few particularly good choices depending on what you want:

  • 🏛️ History: Rome
  • 🎨 Art: Florence
  • ❤️ Romance: Venice
  • 🍕 Food: Bologna or Naples
  • 👗 Fashion/design: Milan
  • ☕ Elegant, less touristy feel: Turin
  • 🌊 Mediterranean/Sicilian experience: Palermo

And don't overlook smaller cities: Lucca, Verona, Parma, Modena, Perugia, Lecce, and Trieste can be excellent additions depending on your route. Italy's tourism board specifically highlights these among its notable cities.
